While he is Commander-in-Chief of the armed forces, that is considered a civilian office. He commands the military, but is not part of it. Considering serving in the armed forces is a prerequisite for being awarded the Medal of Honor, I don't think his plan will work. What about the Presidential Medal of Freedom, though? Those can be given to civilians, and it has "presidential" right there in the name. Nobody is more presidential than Donald Trump, believe me.
Didn't they award Charles Lindbergh the Medal of Honor even though he was a civilian? Of course the criteria might well have changed since the 1920s.

I don't think we are quite there yet.. But. IF President Trump is reelected in 2020. IF he is not impeached and removed from office in his second term. IF he doesn't suffer some major health problem that forces him out of office. Then I can see a strong possibility that we get to the point of needing a 25th Amendment solution sometime during a Trump second term. Not that Pence is my favorite choice but he's all we've got.
painfully humorous perhaps. But yeah, one of the things you might not want to joke about is the medal of honor. Sort of an important award and generally a serious occasion because it's very common to get one posthumously. They don't pass them out like candy that's for sure.

If this had been a ceremony giving out a posthumous award, humor would've been grossly inappropriate. As it is, it was just Trump acknowledging an MoH winner in the crowd in a playful way.
