Such as him being unable to conduct any kind of business without index cards in front of him, which he would read from verbatim, and ignore everyone in the room as he did so. Or him mildly responding to something a visitor excitedly said to him, and then later rushing up to that visitor, and gushing to them with a similar story. These, and many other stories by people who knew, and loved him, make it unlikely that he wasn't mentally impaired by either Alzheimer's or conditions arising after being shot. Just being put under anesthetic at his age, after the shooting, could have been enough to impair his mental abilities for the rest of his life. The disease is not something he, or his family, deserved, but it is what happened to him. One of my grandfathers was clearly suffering from Alzheimer's before he died, but nobody in the family was willing to admit that a man who went from being intelligent, to somebody who resembled Dustin Hoffman in Rainman, had a problem. Nobody wants to admit that someone they loved is gone, even though their body still lives.

Though the patient was unable to accomplish day-to-day things like feeding or bathing himself without help, he was perfectly able to dress himself up and give a fantastic performance on stage to audiences who never suspected he had Alzheimer's. If you're someone who's done a specific job for decades, even if your brain is nearly destroyed by Alzheimer's, you can perform that job well.
It was several years after he left office before anyone even suspected he was having issues. He wasn't diagnosed with it until 1994, two years into Bill Clinton's first term.
