I've nevere been to Japan, and I know there are many people here that have and would know far more about this than me, but from I can recall, the Royal family only exist as a Status Symbol. I don't think they have any real political power over anything. The First Lady in question is still in a position to make decisions, but still falls under ranking to the royal family itself!

You're right. After WWII. The US abolished all imperial power and scaled back the powers of the emperor even worse than the British scaled back the powers of the monarch under the Magna Carta. IIRC from college, the Japanese emperor has NO reserve powers whatsoever. So they basically took everything except his house, clothes, and title. He's simply Emperor of Japan. The US kept the royals around because MacArthur thought they'd be great puppets for propagandizing Americanization. That being said though, as far as "precedence among the people" go, I think they're still considered "first" amongst the people, including the president, government, et al. On another unrelated note, Hirohito should have committed seppuku or something rather than allowing himself to be used like that. (In my humble opinion of course).
