She's shopping for a younger man, even if she doesn't know it. If she got the tits done and he hadn't pulled this move, I would have given the marriage a year at most.

Obviously, she's doing it for herself. He is not crazy to prefer natural tits (though I must disclose, I've never played with natural but small -- natural but medium to large are better than fake of any size). He is a control freak, but he may also rightly understand that the tits represent a larger problem in the relationship. I have a friend who was divorced within six months of his wife getting the silicon pillow inserts. In his case, she did it hoping that tits would save a bad marriage. Yeah, that's always what a marriage is about. I just love that smilie!
Not the husband's decision. He may either accept her choice to mutilate herself, or leave her. But it sounds like she should have left him a long time ago, and the fact that she chooses to stay and fight with him over this makes me question her mental state. Maybe she can't pay for them herself.
